NASF insured Sports Officials receive the following benefits:
$2,000,000 General Liability; $100,000 Excess
Accident Medical Benefit with $250 Deductible;
$5,000 AD&D.

Limits & Rates
$2,000,000 General Liability Per Occurance/Unlimited Aggregate
$250,000 Sexual Abuse/Molestation Per Person/$500,000 Policy Aggregate
$100,000 Excess Accident Medical Benefit ($250 Deductible)
$5000 Accidental Death & Dismemberment Benefit

Rates are $40.00 per sport. EXCEPTION: $45.00 total if officiating both Softball and Baseball

MEDICAL EXCLUSIONS: No benefits will be paid for a loss caused by or resulting from:

1. dental treatment, except as a result of injury to sound, natural teeth;
2. services or treatment rendered by a doctor, or nurse who is the insured or a member of his/her immediate family;
3. war or any act of war;
4. any intentionally self-inflicted injury;
5. charges that are not usual and reasonable for services in the geographic area where performed;
6. sickness or disease. "For a complete list of all exclusions, please refer to the Policy".

Note: Written notice of cliam must be given to Us within 20 days after the covered loss occurs or begins or as soon as reasonably possible. "You will receive claim forms when your certificates of insurance are issued."
